The Empowered Fertility Program will empower you and your partner on your fertility journey.

Terms of Service:

Services

The primary services provided during this program are Coaching, Consulting, and Mentoring. If you are interested in having in-depth therapy services, please seek the services of a counselor or therapist in your area.
 

Diagnosing

While Dr. Squires understands medical and mental health diagnoses and can work with your presenting symptoms, she will not provide any formal diagnoses. 
 

Insurance

Encourage and Empower Wellness does not accept insurance for any services.  The program you have signed up for is a combination of several types of services that are not covered by insurance companies. In addition, since no diagnoses will be given, no diagnostic codes will be provided for insurance reimbursement.

Payment

Full payment or agreement to multiple installments is required at the time you register for the coaching program. All major credit cards and debit cards are accepted as forms of payment.

Private Information/Confidentiality  

You will not be asked to share your address, social security number or insurance number. Your address will be required only when making payment. All of your information is confidential and will not be shared with anyone outside of the Encourage and Empower Wellness Team, which includes Dr. Brenna Squires, Edmund Squires, and administrative support staff.  As a licensed provider, Dr. Squires is bound by law to report safety concerns (e.g., abuse to a child, abuse to the elderly, threat to kill another person, etc.). Any concerns will always be discussed with you prior to reporting.

Report of Suicidal Thoughts

As previously mentioned, Dr. Brenna Squires and Edmund Squires provide coaching and not counseling. If you report having thoughts of suicide, you will be referred to your therapist and the program may be paused. This pause is to help you receive the best help possible! You will not be able to receive the most out of this program if you are contemplating suicide. Suicidal thoughts signify a deeper issue that needs to be processed prior to proceeding on the path.

No Show and No Refund Policy

To ensure you are fully committed to the program, there is a no refund policy. There are no exceptions to this. If you change your mind, and decide not to participate, you are encouraged to gift the program to a family or friend. Once your payment processes, you have officially enrolled in the program and you have agreed to the no refund policy. Your investment covers the entire program, coaching sessions and materials. If you made a partial payment (monthly payment option), the next payment is due on the same date each month. The date for the monthly payment is 4 weeks after the prior payment. Thank you for your understanding, integrity, and promptness.

Concerns

If you have any questions or concerns, it is important that you share this your coach. Confrontation, in any capacity, is part of the process of building healthy relationships. This will be modeled throughout the coaching program. Bringing any and all of your reactions, resistances, or challenges openly will make all the difference in the level of work you accomplish. Please note, your coach cannot address concerns that she is unaware of.
